AHRQ Seeks Comments to Improve Care for People Living with Multiple Chronic Conditions

Does your organization have information that could help improve care for people at risk for or who are living with multiple chronic conditions?  AHRQ is seeking comments on innovative models of care, promising strategies, and solutions to improve care of people with multiple chronic conditions across a variety of care settings (e.g., health systems, primary care, home, and other ambulatory practices).  Don’t delay – Comments are due by November 15.  Access the Federal Register notice for more information and submit responses to MCC@ahrq.hhs.gov.
