The United States Government Accountability Office (GAO) has issued a letter citing all of the open recommendations it has made that the United States Department of Health and Human Services (HHS) has failed to implement. GAO notes that there are 61 such priority open recommendations and that the failure of HHS to act on many of them is a major reason why HHS has programs classified as high risk, including Medicaid and Medicare.
