NYSDOH Issues Reminder on Joining the Age-Friendly Health Systems Action Community

The New York State Department of Health (NYSDOH) has issued a reminder to all health systems and health care providers to register for the New York’s Age-Friendly Health Systems Action Community, an initiative led by the Health Care Association of New York State (HANYS) to transform the care delivered to older adults in New York State, with support from the John A. Hartford Foundation, the Institute for Healthcare Improvement, the Health Foundation for Western and Central New York, the Fan Fox & Leslie R. Samuels Foundation, as well as the New York State Health Foundation.  Through the New York State Action Community, HANYS will connect participating teams from across the State to accelerate adoption of the 4Ms — What Matters, Medication, Mentation and Mobility framework into their practices.  The Action Community will provide virtual trainings by expert faculty and monthly Webinars to test and implement changes that can improve quality of care for older adults and incorporate age-friendly concepts into care delivery.  The Upstate Cohort of the New York State Action Community is launching in November 2020, with the Downstate Cohort launching in February 2021.  Health systems are not limited by geography and can sign up for either of the Action Communities that meets their needs.  To join the over twenty organizations that have already signed up for this exciting, ground-breaking project, you can also register by clicking here.
